UP6723001 Bachelor of Nursing with Honours Programme, Academic Session 2026/2027

GUIDELINES AND INFORMATION FOR THE SCREENING PROCESS BACHELOR OF NURSING WITH HONOURS PROGRAMME, ACADEMIC SESSION 2026/2027

The screening process for this programme consists of two stages:

1. Online submission of essay and required documents
2. Face to face interview

Important Dates and Candidate Responsibilities

 27 April 2026 

Candidates are required to download and complete the following documents:

1. Essay Question Form

2. Health Examination Form

Candidates must complete a medical examination at University Health Centre, Government Health Clinic, or Government Hospital. The examination must be certified by a Medical Officer at the respective facility.

OR

If candidates complete their blood tests and chest X-ray at a private clinic or hospital, you are required to present the results to a Medical Officer at a University Health Centre, Government Health Clinic, or Government Hospital for certification.

3. Declaration of Undertaking Form

27 April 2025 hingga 19 Mei 2026 @ 11.59 midnight

Candidates must upload their completed essay and supporting documents via the following link:

Documents to be uploaded (in PDF format):

1. Essay Sheet*
2. Candidate’s Identity Card*
3. Birth Certificate*
4. Parent/Guardian Identity Card*
5. SPM Certificate*
6. MUET Certificate*
7. STPM / Matriculation / Foundation / Diploma Certificate
8. Final Academic Transcript
9. Completed Health Examination Form - need to be uploaded once completed (candidate can edit respond on Google form)
10. Signed Declaration of Undertaking Form*

*Compulsory document that the candidate needs to upload. Please leave the section blank for the certificate/transcript that is not ready yet

The date and time of the interview for shortlisted candidates will be announced via the registered email. Please check your inbox as well as the junk/spam folders.

 Successful candidates who are scheduled to attend the interview session must bring all required documents as specified in the checklist.

1. Candidates who do not complete any part of the screening process will be considered ineligible for admission.
2. All forms and documents must be prepared and submitted before the deadline. Late submissions will not be considered.
3. All assessments and decisions made by the Faculty are FINAL. No appeals will be entertained.
